This template will create single or multiple file folder labels. It is formatted to fit on the Avery 5366 Labels.

There are text fields for:

• Two lines for descriptive text

• The type of information that is in the folder (e.g., Correspondence)

• Client Matter Number or the File Number

• The initials of the creator
